To whom it may concern: Good day! I,Florante M. De Borja, would like to request another replacement copy of my NC 2 in Automotive Servicing due to lost. I'm already assessed in this Competency Certificate with an Assessmentt Center: First Gear Technical Institute & Assessment Center Inc. .I hope for your kind and urgent response regarding this for I urgently needed it for work.

June 12, 2022 Dear Florante De Borja, Greetings! Thank you for your request dated Aug 10, 2020 08:37:28 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Letter of Request for Replacement Copy of Lost National Certificate. Response to Request Your FOI request is approved. Here are the STEPS for the replacement of loss NC: REPLACEMENT FOR DAMAGE, LOST and CHANGES in the NATIONAL CERTIFICATE (NC)/CERTIFICATE of COMPETENCY (COC) A. LOST NC/COCs STEP 1: Go to the TESDA-issuing office of your NC/COC and apply for replacement. STEP 2: Submit the following documentary requirements: 1. Letter of Request 2. Duly notarized Affidavit of Loss 3. Picture (1 pc., colored, passport size) STEP 3: Claim your NC/COC on the scheduled date given by the TESDA issuing office. I also attached the response in PDF File of the information you requested. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Sec. Isidro S. Lapeña at odg.tesda.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
