Hello po Ako po si Rey Anthony Vergara Balber, nagtapos po ako sa Basilan National High School- Night sa Isabela City, Basilan po Ang teacher ko po ay si Mam Mariel Flores noong 2018 po ako grumaduate Nag a apply po ako ngayon dito sa Manila kailangan ko daw po yung School Certificate Nag chat po ako sa aking teacher pero hindi na daw po nila makita yung certificate ko Pwede po ba akong mag request sa inyo salamat po kailangang kailangan lang po para makapag start po ako Salamat po ulit Ito po ang email ko: [email protected]

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
