In reply, kindly provide the following details/information: 1. Name of your Employers for Calendar Year 2020 to 2022; 2. TIN of your Employers; 3. Registered Address of your Employer. The concerned will be endorsed to the Revenue District Office (RDO) where your employers are registered. They will provide a copy of your requested documents if your employer has filed in your behalf. The RDO might be requiring from you a copy Annex F duly received by the RDO and was submitted by your employer to them. During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 31, 2023 08:47:24 AM, your request will be closed.
