1701 or BIR Form 1701A?
2) What is the Complete Name indicated in the Income Tax Return you filed?
2) What is your Tax Identification Number (TIN) as indicated in your filed
return?
3) What Revenue District Office No. did you file your BIR Form 1701 or BIR
Form 1701A?
4) Did you file your ITR Online (thru eBIRForms facility) or over the counter?
