You just have to submit a Signed Letter Request for the purpose. However, if you are referring to BIR Form No. 2316, the same shall have to be requested from your EMPLOYER since the same is the One Mandated by Law to Prepare and Provide to the Concerned Employee/s pursuant to Section 2.83.1 of Revenue Regulations (RR) No. 10-2008.
