Hi, I would like to ask the logs for the appointments for passport application/renewal from May 1 to June 14 that includes the following information 1. ID of the appointment - Identifier for unique booked appointments for passport application and renewal 2. Date Time of the appointment - Date with Time in PHT when the appointment was completed/booked. 3. IP Address - the IP Address of the computer used during the booking. This is used to trace if the appointment system is being rigged or somebody booking multiple transactions using one computer. 4. DFA Site - The location where it is booked.

Dear Mr. Loable, We acknowledge your Freedom of Information (FOI request for the logs of appointments for passport application/renewal made from 01 May to 14 June 2018 that includes the following information: 1. ID of the appointment- Identifier for unique booked appointments for passport application and renewal. 2. Date and Time of the appointment- Date with Time in PHT when the appointment was completed/booked. 3. IP Address- the IP Address of the computer used during the booking. This is used to trace if the appointment system is being rigged or somebody booking multiple transactions using one computer. 4. DFA Site- The location where it is booked While our aim is to provide information whenever possible, in this instance please be advised that the information falls under the inventory of exceptions to the right to access to information. Section 4(a) of the Memorandum from the Executive Secretary on Executive Order No. 02 s. 2016 states that the following is exempted from FOI: “Information of a personal nature where disclosures would constitute or clearly unwarranted invasion of personal privacy, personal information or records, including sensitive personal information, birth records, school records, or medical or health records…” “However personal information may be disclosed to the extent that the requested information… shall not meddle with or disturb the private life or family relations of the individual…” The need to keep personal information inviolable is recognized in international law as well as in the inventory of Exceptions to Executive Order No. 02.
