Your Request You asked for INVOLUNTARY SEPARATION CERTIFICATE. Response to Request This is to inform you of the process and requirements by which to apply for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION for SSS issued by DOLE-NCR. An employee - Local or Overseas Filipino Worker (OFW) who was involuntarily separated by their employer may file their application for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION at the DOLE-NCR website thru the e-filing services tab by clicking this link: https://docs.google.com/forms/d/e/1FAIpQLSfhNqxP-qEiWAZUmjuTK8ayWHRxFcEfZeJn3kLmv2zDSrAiYw/viewform The requirements for the issuance of the certificate of involuntary separation for uploading online are as follows: a. valid email address (google account) b. at least one (1) valid I.D. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Secretary Silvestre H. Bello III at DOLE Building, Muralla St., Intramuros, Manila. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
