Greetings in the name of the Lord!

I would like to request and/or inquire for any copy of following documents for me to get a certification in our Municipal DILG office to be used as reference to credit my years of service as Sangguniang Kabataan Chairman in our Barangay from August 15, 2002 to November 30, 2007 at Barangay Poblacion 3 Masagana, Balabac, Palawan Region IV-B, for my service to the Philippine National Police. 

         a. Oath of Office and/or
         b. Barangay Official Information Sheet
