I will be using this document for immigration purposes in Canada.
The inclusive dates are as follows.
May 17, 2012 to January 21, 2018 Resignation effectivity 01/22/2018
January 18, 2000 to April 1, 2005 Resignation effectivity 04/02/2005

Thank you for your request dated Oct 12, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.
We received your request on Oct 12, 2022 and will respond on or before Nov 02, 2022 04:29:17 PM, in accordance with the Executive Order's implementing rules and regulations.
In connection with your request through eFOI Portal pertaining to the request for a copy of BIR Form 2316 as a Substitute Income Tax Return for Taxable Year/s 2000 to 2005 and 2012 to 2017.
Please be informed that such request cannot be granted by the Revenue District Office (RDO) since the aforesaid BIR Withholding Tax Certificates were issued in 2000 to 2005 and 2012 to 2017 and records of the RDO on those periods are already unavailable since we follow strictly the five (5) year retention period based on existing revenue issuances of the BIR (Revenue Memorandum Circular 73-2008).
Hence, you may ask your previous employer for the copy/ies of your Certificate of Compensation Payment/ Tax Withheld (BIR Form 2316) because all taxpayers are required to preserve their accounting records for the period of 10 years in accordance with the Revenue Memorandum Circular No. 29-2019.
For Taxable Year 2018, we have endorsed your request to Revenue District Office (RDO) No. 38-North Quezon City for appropriate action.

In particular, it would be helpful to know, if your employer has submitted the Annual Alphalist of employees and that of Annex F you will provide as proof, to be able to certify your Income Tax Return. I refer to my letter of Nov 11, 2022 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Feb 03, 2023.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office.
