If the request is made through a representative, please be guided of the following: 1. In addition to the scanned copies of at least two (2) valid government I.D.s containing the photograph and signature of the requesting party/principal (Mr. Yap); 2. We need to have the scanned or soft copies of the written authorization and/or Special Power of Attorney (SPA) signed by the principal; and 3. Scanned copies of at least two (2) valid government I.D.s containing the photograph and signature of the representative (per Section VII-Standard Procedure for eFOI Requests, Section 21.3 (a) (b).
