I am Ronnie Gabalda, President of Medical Mission Matters Philippines, a duly organized charitable organization under existing Philippine laws, doing surgical missions on patients with Cleft deformities all over the Philippines.
We have an American volunteer in our organization, a certain John Corey aka Jan Roeder Polhemus, whom we required to submit his credentials since last year but which he failed to comply to date.
In this regard, we respectfully request from your good office for a certification regarding the immigration/visa status of the said John Corey aka Jan Roeder Polhemus.
