Hi, I would like to request IMEI BLOCKING or permanently block my phone. I lost it on August 3,2020 my phone is REALME 5pro with lMEI number 865071046571118 .

Your Request You asked for IMEI BLOCKING REQUEST. Response to Request Your FOI request is approved. In reference to the above request, may we advise you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM. However, due to the pandemic situation, before you proceed to the office we encourage you to call first at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form (download form at www.ntc.gov.ph) 2. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
