Hello! I would like to report my lost cellphone. -OPPO CPH2113 Reno 4 Space Black with Imei number 1: 869474057222736 Imei number 2: 86947405222728 S/N: 85033d4d It was stolen on may 1, 2021 and we cannot trace it in find my device because I forget what was may email on that cellphone. Please help me to block it, so that it will be useless to theft. I'm still paying it because it was a loan 🥺 Thank you so much.

May 18, 2021 Dear Myla Mercader, Greetings! Thank you for your request dated May 18, 2021 01:36:41 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Imei Block for my stolen phone. Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information or email at [email protected] . The following are the requirements: 1. Duly accomplished and notarized AFFIDAVIT OF OWNERSHIP AND LOSS WITH UNDERTAKING [Form No. NTC 1-24] Note: The Affidavit of Loss must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1.
