April 14, 2021 Dear Ken Ybardolaza, Greetings! Thank you for your request dated Mar 20, 2021 04:05:41 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for IC Trad & VUL Exam Results on March 09, 2021. Response to Request In relation to your FOI Request, please be advised that you may visit the Insurance Commission-Licensing Division to request for a Certification of Examination Result. In order for the Licensing Division to process the said request, you will be required to submit a request letter indicating examination details such as date, place and the type of examination taken, as well as pay a certification fee amounting to Php500.00. For further inquiries you may contact the IC Licensing Division at Tel. No. 85238461 loc. 111. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by email at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
