Hi! Please help me about my lost vaccine card, I badly needed it for my school. Hoping that you can help me reissue my vaccine card.

September 7, 2023 Dear Anabel Albino, Greetings! Thank you for your request dated Aug 18, 2023 12:06:58 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request on the reissue of your vaccine card, it can be requested from the Municipal Health Office where you have been vaccinated. Thank you.
