Hello im requesting to IMEI block my IMEI for good and because all my account and messages are in my phone that was stole in SM Rosales sunday morning (september 27, 2020) and this is my IMEI and phone's name Huawei y6P, IMEI1:867962052649426 IMEI2:867962052649please block it

October 1, 2020 Dear Angelo Deguzman, Greetings! Thank you for your request dated Sep 30, 2020 08:43:45 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request: You asked for I lost my Phone. Response to Request: Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
