I would like to request for the yearly self-assessments and actions plans of Region 1 hospitals as part of the mandate of AO No. 2013 - 0014.

If available, I would like to request for the indexes for the years 2013, 2014, 2015, 2016, 2017, and 2018.

These will be used as primary sources when analyzing the vulnerabilities and capacities of Region 1 Hospitals.
