I am writing this request for historical data from 2012-2022 of available climate data gathered from the UPLB National Agromet Station. The data will be used for hydrological modelling for my undergraduate thesis. Rest assured that proper citations will be done.

This refers to your request dated 07 February 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.  
You asked for information on the historical climate data in Los Baños, Laguna.
Since you did not provide Form F which needs your signature and should be administered under oath as required under the UP FOI Manual, we regret that we are constrained to DENY your request.

Article III Section 7 of the 1987 Constitution, which is the basis for the requirement under the UP FOI Manual, states:
