I am a fifth-year student from Manuel S. Enverga University Foundation I am currently conducting an Architectural thesis Research and Design 9 as a course requirement for the bachelor of science in architecture entitled "A Proposed CALABARZON Memorial Museum.".

In line with this, I would like to request for the following documents:

- Materials Testing Report
- As-built Survey
- Conservation Assessment Report
- Evaluation Report,

Since you did not provide Forms E and F, with the signature of your professor as the person administering the oath, both being required under the UP FOI Manual, we regret that in the meantime we are constrained to DENY your request.
