Dear DepEd, May I request please for a Certified True Copy of my High School Diploma and Form 137. I took my high school from 2005 to 2009 at Young Rac Highschool in Old Sta.Mesa, Manila. Appreciate your response to this request. Young Rac highschool address is 4427 Old Sta. Mesa St, Barangay Sta. Mesa, Sampaloc, Metro Manila. this is school is now named St. Lino highschool

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
