This refers to your eFOI request wherein you asked for copies of pag-IBIG Fund salary or calamity loan vouchers for payroll deduction purposes from 01 January 2018 to 16 May 2019 of the following Tema Design Philippines, Inc. employees: 1. Damaso, Allan Pernina 2. Garcia, Aries Boadilla 3. Tiad, Juliet Coscos 4. Ngayawen, Rosa Bag-ay Please be advised that Pag-IBIG Fund does not issue individual vouchers of each borrowers since we are now using cashcards in the release of loan proceeds. The procedure is that our Billing and Collection Unit sends billing statements to the address of the employer as written in the loan application form. Based on our records, the aforementioned employees stated in their duly signed application agreement that he/she authorized Tema Design to deduct their loans from their salary. Furthermore, the office address written in the application agreement is 251 Sitio Macabale, San Francisco, Concepcion, Tarlac. Thus, the billing satements were sent to that address. However, our Billing and Collection Unit informed us that said billing statements were being returned for the reason that the addressee has moved out from the indicated address. Hence, may we suggest that your billing statements be sent thru email. Kindly coordinate with our Billing and Collection Unit at [email protected]. In the meantime, we can provide limited statement of accounts (SA) of the aforementioned employees wherein details of the loan are indicated – loan amount, monthly amortization, date granted and the amortization period.
