You requested the following: (1) SAC Number of your mother-in-law, Ms. Genia P. Eridiano.
 
Response to your request

Unfortunately, this is not a valid Freedom of Information (FOI) Request. Thus, we are constrained to deny your request.

However, you may contact the DSWD Social Amelioration Program Hotline Numbers at telephone  number 16545 or by cellular phones through calling 09474822864 (Smart), 09162471194 (Globe) or 09329333251 (Sun) or texting 09189122813 or by e-mail at [email protected] to inquire about your concerns and/or report irregularities about the Social Amelioration Program brought about by the COVID-19 pandemic.

For a copy of the SAC Form of your mother-in-law which contains her SAC Number, you may inquire with your barangay since they may have records since it was the barangay that collected said forms.

Your right to request a review
