I would like to request my Form 138 for my study application in Canada. My parents went to my high school admin but they said they coudn’t find my file because it’s been more than a decade since I graduated there. I really need it as soon as possible because the enrollment will start days from now. And I have all the documents needed except for my Form 138. Please I hope you can help me. Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) that has jurisdiction over the school.
