Hi. I am Carla Jane C. Timbal. I would like to request a copy of my grade 11 Form 138/Report Card. My mother lost it and we cannot find it anymore. I can't apply for the college admission since it is required and I don't have it. We are finding ways to have a copy of it and we saw this site. I am afraid I cannot be admitted to any university since my grade 11 report card was lost. I am hoping that my request will be granted. Thank you. Here's the link to my previous school.
