I would like to request a copy of my form 138. I have contacted my previous school and they are not able to produce any more copies. I lost my form 138 due to housing issues and I am in need of it for my 1st Semestral Clearance in College.

You asked for Form 138.
Response to Request
This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
