Hi, I would like to request my form 138 when I was in grade 12. Its for part time Job application, I need the copy of my official grades as a proof that I can do the job very well since it involves tutoring. I'm not able to go my school to request for a transcript of records due to the location I'm currently in. I would blur/cover my personal infomation that are showed in the form. I just need the grades. Thank You very much!

Your Request You asked for Form 138. Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) which has jurisdiction over the school.
