Good day Ma’am/ Sir! I am Kenneth Jane O. Yunzal. I would like to request for my form 138, in my previous school in high school at Mahaplag National High School, Barangay San Isidro, Mahaplag, Leyte.
The problem is I already took my card in 4th yr high school and was submitted to Greatways Technical Institute in Makati City, where I first studied my college. But unfortunately, the school was permanently close and many of us student there wasn’t able to get our papers anymore. I would like to ask help on how I can be able to get a copy of my Form 138 in high school.
