Hi! I would like to request copies of my Form 137/ TOR or Junior High School Card/ Report Card (Grade 10). It is required po for my college and scholarship admission, it was lost po since palipat lipat kami ng bahay. My Junior High School’s name is General Ricardo Papa Memorial High School, S.Y. 2018-2019. I tried na po mag request sa school pero wala na raw po silang copy ng report card. Also tried to request the Form 137 sa SHS ko pero di raw po nila ito mabibigay. Thank you!
