I am Jade Veronique V. Yap. I graduated from Senior Highschool under the Humanities and Social Sciences strand in PUP-Manila in 2018.
As a requirement in my current school, University of Santo Tomas, I have to submit my Form 137 from PUP. I tried using the Online Document Request (pup.edu.ph), but it seems that senior high school is not in the list.
May I know how to request the following document?

You asked on how you can request for Form 137.
Response to Request
Your FOI request is approved. You may email your request/inquiry at Email [email protected]. Please be informed that starting February 22, the Polytechnic University of the Philippines Sta. Mesa Campus will implement the PUP Visitor’s Appointment Scheduling System (VASS).
With PUP VASS, all visitors and clients of the University should have an approved appointment before being permitted to enter the University premises. To request for an appointment, visit https://apps.pup.edu.ph/appointment
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to VP Pascualito B. Gatan at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
