To whom it may concern, I would like to request to know my average from my Grade 8,9 and 10. As I have difficulty in finding my cards, I didn't intend to loose it but it is because we transferred to a new house. The high school DepEd id number is 406446, I attended high school in Colegio de San Bartolome de Novaliches. I passed the form 137 before in my current school STI College Novaliches, but they still want to know my grades from me. They requires a fee if I would like to request a copy for it, but since I have no money with me, I am trying to find other ways to know it. Tia and pls text

Your Request You asked for Form 137 Grades 8,9 and 10. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Should you have further clarifications, you may directly seek the assistance of the concerned school administration where you spent your elementary and/or secondary education and/or the nearest Schools Division Office (SDO).
