Hello, My former elementary school keeps issuing Form 137s with spelling mistakes or typographical errors. The errors are not in my personal information, but in other text fields like school name or attendance record. I asked them to correct it multiple times, but they continue to make errors and commented that "I ask for too many corrections" and that "These small mistakes dont matter". Do small spelling mistakes in fields like school name or the attendance record (for example) make the document invalid? Who should I contact to check the validity of my Form 137?

Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
