Good Day Ma'am/Sir, I am Hannah Rexeth B. Miranda graduated secondary education in Agoncillo Montessori School batch 2014-2015. I just want to ask if it is possible po to request a copy of my form 137 thru email? I am currently processing my documents for the application of graduation. And the former staff of our school informed me that the files are surrendered in the DepEd division office of batangas since our school was closed. Thank you and God Bless.

Your Request You asked for Form 137. Response to Request Please be informed that the Department of Education (DepEd) Central Office (CO) does not have copy of learners’ school records. Hence, you are advised to get certification from the school where you graduated and bring it to the Schools Division Office (SDO) for a certified copy, since original school records are issued once only. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office (RO) for certification, authentication, and verification (CAV) purposes.
