Hi! I am Mark Saliva, and I am requesting for a copy of my form 137. I am a graduate of Sta. Lucia National High School in Dolores Quezon, batch 2008 - 2009. I already graduated my bachelors degree and the copy of my form 137 is needed for my international post graduate diploma requirement.

Your Request You asked for Form 137. Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) which has jurisdiction over the school.
