Magandang araw po! Ako po ay humihingi ng kopya ng aking form 137 upang gamitin para saking layunin na mag pasa ng application sa ETEEAP. Nais ko pong ipagpatuloy ang aking pag aaral at ang aking choice ay ang programang ETEEAP para sa tulad kong nagttrabaho at may sarili ng pamilya.Wala po akong sapat na oras upang umuwi sa dating probinsya ma aking pinag aralan dahil bukod sa malayo po ay may trabaho din po ako dito sa Manila.Wala din po akong makontak sa aking dating Paaralan.. At wala din po akong kamag anak sa lugar na yun upang mapakiusapan dahil andito na po lahat sa Manila. SALAMAT po

Thank you for your request dated Sep 29, 2021 01:08:42 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Form 137. Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) that has jurisdiction over the school where you graduated.
