Good Day! I am Hassanal R. Guro and I am requesting a copy of my Form 137 from Mindanao Islamic Computer College, where I graduated high school from 2010-2011. They said that I must request directly to the DepEd since this school was already demolished and the marawi area status is still ground zero. l kindly furnish the said record because Form 137-A is a needed requirement for my Job application abroad. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
