February 14, 2021 Dear Lloyd Ryan Buan, Greetings! Thank you for your request dated Feb 11, 2021 06:05:01 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Form 137. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing or visiting our office through the following: Public Assistance Action Center Unit M-106-A, G/F, Mabini Bldg., DepEd Complex, Meralco Ave., Pasig City 1600 Tel.
