Can you assist to get my Form 137 at Saint Peter Academy Of Kaloocan (a Private School https://www.facebook.com/officialspak92/), I have no contact relatives somewhere at Caloocan City and my school Linao National High School-Davao del Norte (https://www.facebook.com/linaonhs2016/) requires immediate comply of this requirement. Please help me on this problem I want to graduate this year.

Response to Request
We advise you (or your immediate relative/authorized representative) to write a formal request letter addressed to the school head/principal, regarding your request for school records. Then your representative will bring these to the Division Office which has jurisdiction over the school. If this is for overseas purposes, he/she will proceed to Regional office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017)
