Your Request
You asked for Form 137.
Response to Request
Please be informed that the Department of Education (DepEd) Central Office (CO) does not have copy of learners’ school records. Hence, you are advised to get certification from the school where you graduated and bring it to the Schools Division Office (SDO) for a certified copy, since original school records are issued once only. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office (RO) for certification, authentication, and verification (CAV) purposes.
Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
