Good Day! I am James Derrick P. Mabasa Jr. and I am requesting a copy of my Form 137 from Ateneo de Davao University where I graduated senior high school from 2017-2019. They would not allow the electronic transfer of a copy of my form 137 to my US Navy Recruiter. I need the form within 20 days to comply with my enlistment requirements and the documents need to be sent to my recruiter’s email: [email protected] I am not able to retrieve the form on person as I am in the United States. Your prompt action would be highly appreciated. Thank you for your kind consideration!

Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) which has jurisdiction over the school.
