Hi Sir/Madam, I would like to request your good office for the school record of my daughter named Anabela Christina L. Vasquez. We tried to look for her school record in West Fairview High School from Grade 7 to Grade 10 and in Lagro High School where she transferred and started Grade 1, but we really cannot find it. We highly appreciate it if you can help us.

Your Request You asked for Form 137. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
