Your Request You asked for Form 137. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. We advise you (or your immediate relative/authorized representative) to write a formal request letter addressed to the school head/principal, regarding your request for school records. Then your representative will bring these to the Division Office which has jurisdiction over the school. If this is for overseas purposes, he/she will proceed to Regional office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing or visiting our office through the following: Public Assistance Action Center Unit M-106-A, G/F, Mabini Bldg., DepEd Complex, Meralco Ave., Pasig City 1600 Tel.
