Hi! I would like to request a copy of my Form 137 since my High School will not release another copy of my credentials. I will be needing this document for the submission of my application this June 01, 2021 as this is the opening for the college I’ve chose to study international. I need it ASAP as I need to reserve my seat on my program of interest because there are few available slot and it is a first come, firs t serve basis Thankyou!

Your Request You asked for Form 137. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Should you have further clarifications, you may directly seek the assistance of the concerned school administration where you spent your elementary and/or secondary education and/or the nearest Schools Division Office (SDO).
