Good Day! I am Hershey Ruth S. Hernandez I am requesting a copy of my Form 137 from Sain Ezekiel School, Bulihan Malolos City Bulacan, where I graduated high school from 2008-2009. They said that I must request directly to the DepEd since this school was already demolished. It will be appreciated if you will kindly furnish the said record because Form 137-A is a needed requirement of a student’s admission to the school that i will enrolling. I will not be able to graduate if I fail to pass this form. Your prompt action would be highly appreciated. Thank you for your kind consideration!
