Good Day!
I am Kessa Jane Gazo and I am requesting a copy of my son Lance Nithrel Cruz Form 137 from St. Theresse De Avila School Paranaque where he attended high school from 2018-2019. They said that I must request directly to the DepEd since this school was already demolished.
My son needed it next week because the school wants to pass the requirements before june ends. It will use for grade 10 enrollment.
