Hi, I would like to request a copy of my Form 137 from highschool. I was hoping anyone could help me with this. My highschool could not give me a Form 137 because they said it would be difficult as i graduated highschool way back 2010. I would appreciate any help please and thank you very much.

Please be informed that the private schools have internal management policies and operational procedures that can usually be read in the School Handbook or Student Manual, which you have signed upon enrollment of your child, thus it has been understood that you are agreeing with their policies.
However, you have the right to discuss your concern with the teachers, school principal, school administrators, or officials for clarifications. If this is not settled, you may write a formal letter and send it to the Schools Division Office of Quezon City through the following, and look for the supervisor in-charge for private schools:
