Good day. Magrequest lang po sana ng FORM 137 nung HS po, Dahil wala pong record si DepEd (permanently closed yung school since 2012), ang advise po ay humingi ng copy sa College School (Family Clinic), unfortunately mag close na din po yung College school at yung nakausap ko po ang sabi sa CHED na daw po kumuha dahil yung mga ibang record na turn over na kay CHED, Badly need ko po yung FORM 137 for school app.ko po. Sana po matulungan niyo po ako. Need daw kasi ng proof na naka Grad ako nung HS.Nagseset po ako ng appointment kay CHED pero lahat ng DATE ay puno na daw po. Thank you.
