Good day! This is to request my Form 137 and Diploma for General Santos National School of Arts and Trades. These documents will be used for my Canadian Application abroad. I hope that your office will grant my special request. As it was very important and urgent. I am currently living in Silang, Cavite now and unable to go in General Santos because of the current situation. Thank you and more power!

Your Request You asked for FORM 137 and Diploma. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Should you have further clarifications, you may directly seek the assistance of the concerned school administration where you spent your elementary and/or secondary education and/or the nearest Schools Division Office (SDO).
