Good Day! I am Diana G. Concepcion. LRN No. 103221080003. I would like to request a soft copy of my Form 137 and 138 since my school did not take an action into it when I inquired them about my missing documents. If possible, can I receive it by this evening? The deadline is today so I'm just wondering if you could do that. Thank you very much.

Your Request You asked for Form 137 and 138. Response to Request This is to inform you that the Department of Education (DepEd) Central Office does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). Should you have further clarifications, you may directly seek the assistance of the concerned school administration where you spent your elementary and/or secondary education and/or the nearest Schools Division Office (SDO).
