Hello po Pwede po ba magrequest ng form 137-A kasi closed na po permanently yung school ko ng highschool and and advice po sakin sa registar ay mag request daw po ako sa deped. Need ko po kasi yung form 137–A as requirement for graduation po this college. Maraming salamat po.
