February 6, 2021 Dear Mr. ALJOHN MANGONDATO, Greetings! Thank you for your request dated Jan 18, 2021 10:57:02 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA Verification System shows that you applied online on January 5, 2021 was evaluated by OWWA-RWO 10 as PENDING for the submission of the following douments: (1) arrival stamped page in your passport; (2) PSA Certified Birth Certificate of dependent and Certified Grades from the school registrar; and (3) Proof of Loss of Employment on account of COVID-19. Submit the documents to qualify your dependent to the OWWA-EASE. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter.
